New Patient Registration
We are happy to register patients living within the following post code areas:
G13
G12.0
G12.9
G61.1
G61.2
To register with the practice, please simply complete the NHS Scotland Patient Registration document here as well as our New Patient Questionnaire here. Males should complete only pages 1 and 2 and females should complete all 3 pages of the questionnaire. A separate Registration form and questionnaire should be completed for EACH person wishing to register.
Please ensure that you print/save both documents and complete them IN FULL as your registration may be delayed if information is missing.
Once completed, please return both forms by email to ggc.gp40008clinical@nhs.scot
You will be notified once we have received them and registered you as a patient.
PLEASE NOTE THAT, DUE TO DEMAND ON SERVICES, WE ARE PRESENTLY UNABLE TO REGISTER YOU IF YOU ARE ALREADY WITH (or have just left) A SURGERY WITHIN OUR LOCALE.
