New Patient Registration
To register with the Practice, you can call in at your convenience, bringing your medical card with you, along with some form of ID such as a passport or driving licence. Don’t worry if you don’t have this card as we will be able to get most of the details we need from you when you come in.
We are happy to register patients living within the following post code areas:
For all new patients over the age of 5 years, an appointment will be arranged for you to see the Practice Nurse in the first instance. This is a simple check to assess your general health and medical needs.
Further to Covid restrictions, it has now been made possible for you to register with our surgery online to save you from attending the practice in person. To do this, you will require to complete the NHS Scotland Patient Registration
as well as our New Patient Questionnaire here
. Males should complete only pages 1 and 2 and females should complete all 3 pages of the questionnaire. A separate Registration form and questionnaire should be completed for EACH person wishing to register.
Please ensure that you print/save both documents and complete them IN FULL as your registration may be delayed if information is missing.
Once completed, please return both forms by email to email@example.com
You will be notified once we have received them and registered you as a patient.
